Providing caring and sensitive companionship and assistance with meals, errands and transportation, walking and mobility, and mentally stimulating activities such as reading and games is the goal of Platinum Home Health Care’s Alzheimer’s and dementia care programs, with the objective of safety, physical activity, and memory preservation. Platinum Home Health Care is committed to serving this special population and provides services through various delivery mechanisms including Managed Long Term Care insurance (MLTC), Medicaid Managed Care, and through it’s participation in the Nursing Home Transition and Diversion (NHTD) Waiver program. Platinum Home Health Care is a premier provider of services to the NHTD program in the New York City region.
The Nursing Home Transition and Diversion (“NHTD”) Medicaid Waiver is a program that uses Medicaid funding to provide supports and services to assist individuals with disabilities and seniors to reside safely in the community in their own homes. Waiver participants may come from a nursing facility (transition), or choose to participate in the waiver to prevent institutionalization (diversion). The NHTD program is appropriate when informal supports, and other available services are not sufficient to assure the health and welfare of the person in the community.
Through its participation in the NHTD program Platinum provides Service Coordination services and NHTD specific home care services (HCSS). HCSS services are designed for persons with cognitive deficits such as those resulting from Alzheimer’s, Dementia, or stroke.
Service Coordination (SC): The Service Coordinator functions as the NHTD program participant’s advocate in the community. The Service Coordinator’s goal is to increase the participant’s independence, productivity, and integration into the community while maintaining the health and welfare of the individual. The Service Coordinator assists the prospective participant with the enrollment process and coordinate and monitor the ongoing provision of all services in the Service Plan. Services may include the coordination of benefits and entitlements such as Medicaid and food stamps, arranging for medical appointments and rehabilitation services including transportation, and ensuring the participant’s safety via the provision of all other necessary services including NHTD Waiver and non-waiver services.
Home and Community Support Services (HCSS): Platinum Home Health Care recognizes that individuals with cognitive impairments that may accompany a diagnosis of Alzheimer’s, dementia, stroke, etc. often require specialized services in addition to traditional home care. Home and Community Support Services are individually designed home care services that address a need for supervision and oversight in addition to support with any physical care needs. HCSS can provide prompting, direction, supervision and oversight, as well as support in the following areas:
HCSS staff are trained specifically to work with cognitively impaired persons and to address the physical, cognitive, and behavioral deficits common within this population
Independent Living Skills Training (ILST) – Assists the individual in achieving a maximal level of independence in the community. This may include assistance with household management and financial management (bill payment) when natural supports are not available to provide assistance.
Structured Day Program (SDP) – Day program services provided in a group setting with other individuals with cognitive and/or physical impairments to address the promotion of independence and provide socialization and leisure activities.
Community Integration Counseling (CIC) – A counseling service to address the challenges faced by the individual in adjusting to life after a change in cognitive and/or physical functioning.
Positive Behavioral Interventions and Supports (PBIS) – A service designed to address maladaptive behaviors that threaten the cognitively impaired individual’s ability to remain safely in the community. Providers work with the individual, their family, and caregivers to eliminate or minimize maladaptive behaviors.

Platinum Home Health Care provides comprehensive training to educate caregivers. Trainings are developed and conducted by nurses, social workers, and other specialists working with the cognitively and/or physically impaired population. NHTD Waiver Program staff receive specialized training in the physical, psychological, behavioral, clinical, rehabilitation, and social needs of cognitively impaired individuals. Training includes information regarding the understanding and promotion of the individual’s rights, choices, and independence. All of Platinum’s NHTD Waiver program staff are evaluated and certified for their competency in the delivery of care to individuals.
